About Your Visit
Dear New Patient,
Thank you for choosing Westchester Headache Center. It is our every desire to provide you with excellent and timely service. The following guidelines were designed to facilitate this process.
- Please arrive on time for your appointment. We do not penalize patient for arriving late.
- Please bring your current insurance card to the initial consultation. We will complete and submit all insurance papers on behalf of the patient.
- For your convenience our office accepts checks, cash, money orders and major credit cards.
- We will make every effort to accommodate your scheduling needs. In return, we ask that you help us by keeping your scheduled appointments or by notifying us at least one business day in advance if you are unable to do so.

Please download, print and complete the New Patient Registration, Headache History Questionnaire and HIPAA Receipt of Acknowledgement before your appointment with Dr. Silverstein. You can bring the forms with you to your appointment and this will help expedite your waiting time. Thank you!
